The following information outlines the process for submitting the paper based applications
- If unable to complete the online application, download and enter your information into the PDF application form fields
- After completing the form, print the completed application form
- Print the Application Form and all of the additional required forms for the membership type you are requesting.
- For 1st Senior applications: Include the completed application and all additional required forms
- $25 payment for the background check
- Completed 1st Senior Application Form
- 1st Senior Membership waiver form w/ applicant signature
- Two (2) Fully completed / signed Current 1st Senior Member Reference Forms
- For 2nd Senior applications: Include the completed application and all additional required forms
- $25 payment for the background check
- Completed 2nd Senior Application Form
- 2nd Senior Membership waiver form w/ applicant signature
- For Junior applications : Include the completed application and all additional required forms
- Completed Junior Application Form
- Junior Waiver form - signed by the sponsoring 1st Senior
